On http://www.digitalmars.com/d/phobos/std_md5.html - how is that Example section output all fancy like that? I tried it in mine and it came out terrible. Plus, the ddoc documentation page says there's a section Examples, not Example.
Also, the BUGS part isn't exactly a bug; plus, the way it's worded is misleading, as if they're always not unique.

He is using a customized set of Ddoc macros that generate HTML that uses his style sheet  ( http://www.digitalmars.com/d/style.css )

The beauty of Ddoc is that anyone can override the default Ddoc macros with their own flavor and end up with a radically different output document.
