  Cleanup handling of ALLREGS and BYTEREGS.

- move definition from elfobj.c and machobj.c to cs.c
- move initialization from global in elf and machobj to cod3_set[16|32|64]

sorry, pushed to the wrong tree, but was on a non-master branch, so no harm done.  I've already deleted it and will put it through a standard pull request.
